Who is eligible community access support?

NDIS participants who have funding under Core Supports or Capacity Building, especially “Increased Social and Community Participation”, are eligible for Community Access. This support helps people engage in their community, develop skills, and build confidence with the help of a trusted support worker.

Will I always have the same support worker?

Consistency matters. Where possible, we match participants with regular support workers who suit their personality, communication style, and routine. This helps build trust and ensures a more comfortable, supportive experience every time you head out into the community.

How often can I access support?

It’s completely flexible and based on what’s in your NDIS plan and what works for you. Some participants head out weekly, others a few times a week, and some prefer occasional outings. We work with you to develop a routine that matches your goals and lifestyle.

Can I choose what I want to do?

Absolutely. Community Access is all about your goals and interests. Whether it’s attending a group, shopping, trying a new hobby, or just getting out and about more confidently—we’ll support you in doing the things that matter most to you.

How do health professionals refer patients to your services?

Referrals can be made directly by support coordinators, GPs, case managers, or allied health professionals. Our intake team works closely with referrers to understand the participant’s goals, support needs, and preferences to ensure a smooth transition into services. How do you work with families and carers to keep them involved?

Families and carers play a vital role in a participant’s journey, and we keep them informed every step of the way. We offer regular updates, collaborative planning, and open lines of communication to ensure everyone feels connected, supported, and included in the progress being made.

Do you provide transport?

Yes. We can include transport in your support if it’s covered by your NDIS plan. Whether it’s a local outing or a scheduled appointment, we make sure you can get there safely and comfortably—with support before, during, and after the activity if needed.
